The goal of the LIVESTRONG Fertility Discount Program for Men and Women is to increase access to fertility preservation services for individuals who are diagnosed with cancer during their reproductive years. The LIVESTRONG Foundation offers free fertility stimulation medications through EMD Serono and discounted IVF, sperm banking, and egg and embryo freezing services through an ever-growing national network of reproductive clinics and sperm banking partners.
Eligibility Criteria:
- U.S. Citizen or permanent resident
- Annual adjusted income:
- Female: is less than or equal to $115,000 (if single) or $150,000 (if married)
- Male: is less than or equal to 90,000 (if single) or $115,000 (if married)
- Diagnosis of cancer
- Oncologist has determined that the recommended cancer treatments present the risk of infertility or has caused risk to the patient's fertility
- Oncologist and endocrinologist have both determined that the treatments and associated medications are medically appropriate
- Limited to no insurance coverage
- The fertility center has agreed to provide the requested service through the LIVESTRONG Fertility Discount Program
How to Apply

Register and initiate an application through the LIVESTRONG Fertility portal between the hours of 8:30 AM to 5:00 PM (EST) at www.livestrong.org/fertilityapplication. Hours are subject to change.

Complete each section of the online application, with the help of your healthcare team. If you are applying for partner support (men), please have the oncologist complete the oncology form with the male partner information.
